Infertilidad femenina asociada a endometriosis: impacto del tratamiento médico, quirúrgico y reproducción asistida sobre las tasas de embarazo y nacido vivo

La infertilidad asociada a endometriosis requiere distinguir control sintomático y beneficio reproductivo. Se realizó una revisión narrativa crítica de guías, ensayos y síntesis sobre tratamiento médico, cirugía y reproducción asistida. Se diferenciaron embarazo clínico, embarazo evolutivo y nacido vivo, conservando denominadores y fenotipos. La cirugía puede favorecer embarazo en enfermedad mínima o leve, pero la evidencia no respalda su uso sistemático antes de FIV para aumentar nacido vivo; la reserva ovárica requiere especial consideración. Se propone una matriz de decisión compartida basada en objetivos, pronóstico y reevaluación. Se concluye que el tratamiento debe preservar tiempo reproductivo y función ovárica, con expectativas informadas y atención individualizada.
